The European committee ruled that people have the “right to be forgotten” and Google has designed an online form from which you can request for links to be removed from the search results.

After your application, Google will assess your demands to balance your privacy rights with the public’s right to know and distribute information.

To submit the form you have to supply the URL and clarify how the links are associated to you. You must also provide your personal details including a photo ID.

Note, that if google favors your request, they will remove the links in question from search results across the EU and not across the US.
